About Our Preceptors

Our experienced practitioners provide supervision during clinical practice and facilitate the application of theory to practice for students in our pharmacy residency program.

Dr. Johnston received her undergraduate training at Louisiana State University and further went on to obtain her doctor of pharmacy degree from the Xavier University of Louisiana College of Pharmacy in 2017. She completed her PGY1 Pharmacy Residency at Our Lady of the Lake in 2018. Dr. Johnston received her teaching certificate at South Carolina College of Pharmacy and completed her PGY2 Critical Care Residency at Prisma Health Richland, in Columbia, South Carolina, in 2019. She is involved in Residency Advisory Council Subcommittee, Code Blue Committee, Critical Care Operations, Preceptor Development Committee, and Residency Research Subcommittee. Dr. Johnston serves as an APPE Clinical Preceptor for the University of Louisiana-Monroe and Xavier University of Louisiana College of Pharmacy. She serves as the current PGY2 Critical Care Residency Program Director. Her interests include pain/sedation/delirium, code blue response, COVID-19 management, and addiction management in the ICU.

Dr. Jones graduated from Mississippi State University in 2008 with a bachelor of science in biochemistry/molecular biology. She received her doctor of pharmacy degree from the University of Florida. Dr. Jones joined Our Lady of the Lake in 2012 as one of the first PGY1 Pharmacy Residents. She serves on the InPharmation Newsletter Committee, Pharmacy Council and Residency Advisory Committee. Dr. Jones serves as a preceptor for Xavier and ULM students.

Dr. Dastoori received her doctorate of pharmacy degree from Xavier University of Louisiana College of Pharmacy in 2017. She completed her PGY1 Pharmacy Residency at Our Lady of the Lake Regional Medical Center in 2018 where she also completed the University of Connecticut online teaching certificate program. She serves as a longitudinal preceptor for Pharmacy Practice Management which entails staffing. She serves on several committees including the Residency Recruitment Subcommittee and the Residency Advisory Council. She is the past local president of the Louisiana Society of Health Systems Pharmacists and current member. She has dual board certification is pharmacotherapy and critical care. Her interests are critical care, emergency medicine, toxicology, and endocrine emergencies.

Dr. Ezebuenyi received his doctor of pharmacy degree from Xavier University of Louisiana College of Pharmacy in 2015. He completed his PGY1 Pharmacy Practice Residency at University Medical Center/Xavier College of Pharmacy in 2016. He obtained a Scholarship of Teaching Certificate from the University of Kentucky College of Pharmacy. Dr. Ezebuenyi joined Our Lady of the Lake in 2016 and started as a member of the Internal Medicine clinical staff group. He now serves as the Cardiology Clinical Specialist and is a member of the Heart Failure, Electrophysiology, Reduce the Risk-PCI Bleed, Residency Advisory Council, and Pharmacy Resident Research Committees. He is also an adjunct clinical professor for Xavier University of Louisiana College of Pharmacy and serves as an APPE clinical preceptor for Xavier University of Louisiana College of Pharmacy. His practice interests include cardiology, pharmacokinetics, nutrition support, and antimicrobial and anticoagulant stewardship.

Dr. Bridges graduated from Louisiana State University in 2014 with a bachelor of science in biology. She received her doctor of pharmacy degree from the University of Louisiana at Monroe College of Pharmacy. She completed her PGY1 residency at University of Mississippi Medical Center where she also received her Teaching and Learning Certificate from the University of Mississippi School of Pharmacy. Dr. Bridges joined Our Lady of the Lake team as the Surgical Intensive Care Unit Clinical Staff Pharmacist in 2020. She is involved with Residency Advisory Committee and Pharmacy Council. Her areas of interest include critical care, fluid stewardship, parenteral nutrition, code blue response and antimicrobial stewardship.

Dr. Smith received her bachelor of science degree in pharmaceutical sciences from the University of Mississippi and further went on to obtain her doctor of pharmacy degree from the University of Mississippi School of Pharmacy in 2017. She completed her PGY1 Pharmacy Residency at University Health Shreveport in Shreveport, Louisiana, in 2018 where she also received her Teaching Certificate from the University of Louisiana-Monroe School of Pharmacy. She joined the team at Our Lady of the Lake in July 2018 as one of the Critical Care Clinical Staff Pharmacists and transitioned into the Neuro Critical Care Specialist role in 2020. Dr. Smith serves on several committees including the Stroke Leadership and Neuro-Sciences Committee, Residency Advisory Committee, Education Committee, Preceptor Development Committee, Residency Research Subcommittee,  Residency Recruitment Committee as well as has served as South Central LSHP Chapter president for two years from 2019-2021. Dr. Smith serves as an APPE Clinical preceptor for the University of Louisiana-Monroe School of Pharmacy and became the PGY1 Residency Program Director in 2021. Her interest areas include stroke and thromobolitic management, status epilepticus, RSI management, code blue response and toxicology.

Dr. Ledet received her Bachelor of Pharmaceutical Sciences from the University of Louisiana at Monroe and went on to obtain her Doctor of Pharmacy degree from the University of Louisiana at Monroe in 2020. She completed her PGY1 Pharmacy Residency at Our Lady of the Lake Regional Medical Center and received her Teaching Certificate from the University of Connecticut School of Pharmacy in June 2021. After residency, Dr. Ledet joined the OLOL team as an Internal Medicine Clinical Staff Pharmacist. She then transitioned into a Critical Care position and now works in the Medical Intensive Care Unit. She is currently studying for BCPS certification. Her current interests include critical care, emergency medicine, code blue response, and antimicrobial stewardship.

Dr. Kirkwood received her bachelor of science degree in biochemistry from Louisiana State University and her bachelor of science degree in pharmaceutical sciences from the University of Louisiana at Monroe where she then completed her doctor of pharmacy degree. She completed her PGY1 Pharmacy Residency at University Medical Center in New Orleans and completed her PGY2 Critical Care Pharmacy Residency at Our Lady of the Lake Regional Medical Center in Baton Rouge. She joined the Our Lady of the Lake team in August 2021 as a Critical Care Clinical Staff Pharmacist and recently transitioned into the Trauma Neuro Critical Care Clinical Specialist position. She is studying for board certification in critical care. Her practice interests include critical care, trauma, emergency medicine, cardiology and psychiatry.

Dr. Metrejean earned her Bachelor of Science degree in Public Health from The University of Arizona and further went on to obtain her Doctor of Pharmacy degree from The University of Arizona College of Pharmacy in 2018. She then earned her teaching certificate from the UConn School of Pharmacy while completing her PGY1 pharmacy residency at Our Lady of the Lake in 2019. She is involved in the Emergency Management, Residency Advisory Council, and Stroke Committees, and serves as the pharmacy department newsletter Chair. In a teaching capacity, Dr. Metrejean serves as a Clinical Assistant Professor of Medicine for LSU and a Clinical Instructor for The University of Arizona College of Pharmacy. She recently earned her Emergency Neurological Life Support certificate and is seeking board certification in emergency medicine. Her interests include critical care, toxicology, and emergency preparedness.

Dr. LeBlanc received his doctor of pharmacy Degree from the University of Louisiana at Monroe College of Pharmacy in 2017. He completed PGY1 Pharmacy Residency training at Mercy Hospital in Joplin, Missouri, in 2018, where he received his teaching certificate at the University of Missouri-Kansas City School of Pharmacy. He joined the team at Our Lady of the Lake in January 2020, where he practices as one of the Emergency Medicine Clinical Pharmacy Specialists. He is involved in the Stroke committee. He is board certified in pharmacotherapy and his practice interest include emergency medicine, toxicology and infectious disease.

Dr. Parikh received his bachelor of biological sciences degree from the Rutgers University and further went on to obtain his doctor of pharmacy degree from the University of South Carolina in 2017. He completed his PGY1 Pharmacy Practice Residency at the Virginia Commonwealth University Health System in Richmond, Virginia, in 2018, where he also received his teaching certificate. Dr. Parikh completed his PGY2 Emergency Medicine Residency at Rutgers University in New Brunswick, NJ in 2019. He joined the team at Our Lady of the Lake in July 2019, where he practices as one of the Emergency Medicine Specialists. He is involved in several committees including: Stroke, Trauma, Code Blue and the Residency Advisory Council. His current interests include acute pain management, procedural sedation, infectious diseases and toxicology.

Dr. Weithman received her doctor of pharmacy degree from the Ohio Northern University in 2018. She completed her PGY1 Pharmacy Practice Residency at the St. Elizabeth Healthcare in Edgewood, Kentucky, in 2019, where she also received her teaching certificate through the University of Cincinnati Winkle College of Pharmacy Teaching Program. Dr. Weithman completed her PGY2 Emergency Medicine Residency at Indiana University in Indianapolis in 2021. She joined the team at Our Lady of the Lake in July 2021, where she practices as one of the Emergency Medicine Nocturnist Specialists. She is board certified in pharmacotherapy and her interests include trauma, neurocritical care and infectious diseases.

Dr. Schrack received her Bachelor of Science degree at the University of Tennessee at Knoxville and further went on to obtain her Doctor of Pharmacy degree from the University of Tennessee Health Science Center in 2020. She completed her PGY1 Pharmacy Residency at WakeMed Health & Hospitals in Raleigh, NC and her PGY2 Infectious Diseases Pharmacy Residency at Novant Health Presbyterian Medical Center in Charlotte, NC. She received her Teaching Certificate from Shenandoah University in June of 2022. She joined the team at Our Lady of the Lake in August 2022, where she rotates between the LSU Infectious Diseases Consult Service and Antimicrobial Stewardship. She is studying for board certification in Infectious Diseases and her interests include multidrug-resistant Gram-negative organisms, rapid diagnostic testing, antimicrobial therapeutic drug monitoring, and antimicrobial stewardship.

Dr. Ricks received her doctor of pharmacy degree from Xavier University of Louisiana College of Pharmacy in 2013. She completed her PGY1 Pharmacy Residency at Our Lady of the Lake Regional Medical Center in 2014 and joined the team at OLOL as part of the Internal Medicine Clinical Staff upon completion of her residency. Dr. Ricks serves on the Preceptor Development Subcommittee and Pharmacy Counsel Committee at OLOL and has recently taken on the role of Clinical Pharmacy Supervisor.

Dr. Hartenstein received her bachelor of science in pharmacy from the University of Mississippi in 1981 then later returned to school to obtain her doctor of pharmacy degree from Xavier University of Louisiana College of Pharmacy in 1999. She joined Our Lady of the Lake in 2009 serving as the clinical pharmacist for PACE of Baton Rouge, a program for the care of the elderly. Dr. Hartenstein transitioned to the Internal Medicine Clinical Staff group in 2018. She is studying for board certification in pharmacotherapy and her interests include internal medicine and geriatrics.


Clare Olin, PharmD, BCPS

Clare Olin, PharmD, BCPS, Transitions of Care Clinical Pharmacy Specialist

Dr. Olin received her Doctor of Pharmacy degree from University of Louisiana at Monroe in 2019. She completed PGY1 Pharmacy Residency at the University of Mississippi Medical Center and received her Teaching and Learning certificate from University of Mississippi School of Pharmacy in 2020. She joined the internal medicine clinical staff team in January of 2022. Her interests include internal medicine, antimicrobial stewardship, and cardiology.


Investigational Drugs: Anjali Namjoshi, Pharm.D., Investigational Drug Pharmacist

Dr. Namjoshi received her doctor of pharmacy degree from Xavier University of Louisiana College of Pharmacy in 1995 graduating at the top of her class with a 4.0 GPA. She completed her residency training at the VA Medical Center in New Orleans and joined Our Lady of the Lake in 1999. She previously served as the Clinical Pharmacist in Adult Critical Care for 15 years before moving to her current position as the Investigational Drug Pharmacist. Dr. Namjoshi offers an elective rotation in investigational drug service that introduces the resident to various aspects of conducting actual clinical trials involving a pharmaceutical agent. She has served on the Pharmacy and Therapeutics Committee, Nutrition Support Team and the Critical Care Committee as well as adjunct clinical faculty for University of Louisiana in Monroe College of Pharmacy where she received Preceptor of the Year Award in May 2005. 


Affiliate Hospital: Our Lady of the Lake Children's Hospital 

Kristin Howell, Pharm.D., BCPS, BCPPS, Neonatal Clinical Specialist

Dr. Howell received her BS in Toxicology from University of Louisiana at Monroe in 2012, followed by a BS in Pharmaceutical Sciences in 2015. In 2016, Dr. Howell received her doctor of pharmacy, magna cum laude, from the University of Louisiana at Monroe College of Pharmacy. Dr. Howell completed her PGY1 Pharmacy Residency at Our Lady of the Lake Regional Medical Center in 2017. Upon completion of her residency, Dr. Howell took a position as the Neonatal Intensive Care Unit Specialist at Woman’s Hospital. During her time at Woman’s Hospital, she became a certified LEAN Six Sigma Green Belt. As of 2019, Dr. Howell has decided to return to OLOL to join the Children’s Hospital pediatric team as the Neonatal Clinical Specialist. Her practice interests include NICU, nutrition, and antimicrobial stewardship.


Rachel Mathews, Pharm.D., BCPPS, Pediatrics Clinical Specialist

Dr. Mathews received her doctor of pharmacy degree from the University of Tennessee Health Science Center College of Pharmacy in Memphis. She completed her PGY1 Pharmacy Residency training at Kosair Children’s Hospital in Louisville, Kentucky. Dr. Mathews has been with Our Lady of the Lake Regional Medical Center since 2010. She currently serves as a member of the Children’s Hospital P&T, Children’s Hospital Nursing/Pharmacy Safety Workgroup, Children’s Hospital Code Committee and Children’s Hospital Quarterly Quality Committee.
