Our continuity clinic, located at 1805 Jackson Street in Monroe, provides residents with the opportunity to provide longitudinal care that includes the management of chronic diseases with any resulting complications, patient counseling, as well as follow the natural course of illness. Residents will be given the tools to evaluate chronic disease management and deliver preventative health care through educational programs such as articles, lectures, observing clinical instructors, etc. Data will be monitored by the quality department for hospital readmissions, adverse events, or negative outcomes as a result of chronic disorders and this feedback will be provided to the responsible resident, social worker, and their preceptor.
The resident longitudinal experience will not be interrupted by more than 4 weeks, not inclusive of vacation and will account for approximately 30% of their curriculum. There will be one preceptor serving as the Medical Director for the function of the continuity clinic, with additional providers attending as needed. Additional attendings will be a consistent group of physicians working with residents and their patients to ensure strong peer relationships. Scheduling will be conducted mindfully to provide consistency for patients, residents, and attendings.
