New Team Member

Welcome to the FMOLHS family! We’re so excited to work with you and have you as a part of our healing ministry. On this page, you’ll find your benefit enrollment guide and information on how to enroll in benefits.

How to enroll:

  • New team members have 30 calendar days from your date of hire to enroll in benefits and submit complete dependent verification documentation. 
  • We have created a checklist that you can use to ensure you are completing the enrollment process.
  •  Review Benefit Options
  •  Complete Benefit Elections through Oracle Employee Self-Service
  •  Verify Dependent Eligibility
    • Team members are required to submit complete dependent verification documentation for all dependents.
  •  Check Oracle Employee Self Service for Enrollment Confirmation
    • You can verify your enrollment and print a copy of your elections in Oracle Self Service by clicking on Me/Benefits/My Benefits.
  •  Apply for Just Premium (if eligible)
    • Premium reduction opportunity based on federal income guidelines.
