Franciscan PACE

Franciscan Program of All-Inclusive Care for the Elderly, or Franciscan PACE, is a nonprofit health and supportive services program offering seniors an alternative to long-term care placement with centers in Baton Rouge and Lafayette.

The mission of Franciscan PACE is to enable the aging population to live with dignity in their communities. With Franciscan PACE’s assistance, participants can remain in the secure surroundings of their own homes and communities while providing family members and caregivers with much-needed professional guidance, physical relief and emotional support.

Franciscan PACE takes a team approach to care with a diverse, multi-disciplinary team. The team works together to develop individual care plans and goals and carefully monitor progress.

    • Family caregiver support
    • Medical equipment
    • Nutritional counseling
    • Spiritual care
    • Medications
    • Recreational therapy
    • Regular assessment of the participant's needs

    • Personal care services in the home
    • Social services
    • Transportation to and from the center
    • Inpatient and outpatient services contracted with Our Lady of the Lake Regional Medical Center and its affiliates

  • Who is Eligible to Join PACE?

    • Ages 55 or older
    • In Baton Rouge, you must live in East or West Baton Rouge Parish (I can outline this for you)
    • In Lafayette, you must live in these zip codes: 70501, 70503, 70506, 70507, 70508, 70517, 70518 70520, 70529, 70578, 70583, 70584, 70592
    • Medicaid eligible or willing to pay privately equal to the current Medicaid rate
    • Willing to receive all needed healthcare, including primary care and specialist physician services (other than emergency services) from the PACE organization or from a provider authorized by the PACE organization
    • Meeting Nursing Home Level of Care, meaning you need assistance with activities of daily living such as moving about, memory and/or assistance in the bathroom
  • What Will PACE Cost?

    • What you pay will depend on your financial situation.
    • If you qualify for Medicaid services, you will not pay a monthly premium for PACE.
    • Those who do not qualify financially for Medicaid will pay a PACE premium, which equals the current Medicaid rate.
    • There are NO co-payments or deductibles.
    • PACE covers all necessary medical and social services, and the premium rate does not change based on your changing needs.
