If you believe that Franciscan Missionaries of Our Lady Health System and its covered entity affiliates and their subsidiaries has failed to provide these services or discriminated in another way on the basis of race, color, national origin, age, disability, or sex, you can file a grievance with the Civil Rights Coordinator listed below. You can file a grievance in person or by mail, fax, or email. If you need help filing a grievance, you may call (800) 315-9566. If you would like to file your grievance in writing you may contact your Civil Rights Coordinator as listed below.
