Team Member Memorial Screen

Recognize and honor team members who have died within the past year while working at any location of Our Lady of the Lake Health or up to the time when they were no longer able to work due to illness or injury prior to their death by filling and submitting the web form below.

The deceased’s photo and write-up will be added to the Team Member Memorial Screen located on the team member entrance hallway on the first floor of our regional medical center.
