• PACE will be the participant’s sole service provider and guarantees access to services, but not a specific provider.

• Medicaid eligible or willing to pay privately equal to the current Medicaid rate
• Must receive all needed healthcare, including primary care and specialist physician services (other than emergency services) from the PACE organization or from a provider authorized by the PACE organization.
• You might qualify if you need physical help with self-care; have a chronic medical condition that requires physician involvement; and/or have difficulty with memory and understanding and participating in conversations.

• What you pay will depend on your financial situation.
• If you qualify for Medicaid services, you will not pay a monthly premium for PACE.
• Those who do not qualify financially for Medicaid will pay a PACE premium, which equals the current Medicaid rate.
• If you do not have Medicare or Medicaid, you can pay for PACE privately.
• There are NO co-payments or deductibles.
• PACE covers all necessary medical and social services, and the premium rate does not change based on your changing needs.
• PACE participants may be fully and personally liable for the costs of unauthorized or out-of-PACE program agreement services.
