PACE (Program of All-Inclusive Care for the Elderly) is a nonprofit health and supportive services program designed to assist seniors who want an alternative to long-term care placement. The mission of PACE is to enable the aging population to live with dignity in their communities. PACE participants can remain in the secure surroundings of their own homes and communities while providing family members and caregivers with much needed professional guidance; physical relief and emotional support.
PACE takes a team approach to care which revolves around its diverse, multi-disciplinary team. The team works together to develop individual care plans, goals and carefully monitor the progress of each participant.

PACE is a Different Type of Care
PACE is not all about healthcare services. It’s a place to go to meet new friends, participate in fun activities and get your medical care. PACE is designed to meet all these needs: medical, social and emotional. While most care is delivered in the adult day health center during the day, care is coordinated 24 hours a day. PACE’s day-to-day monitoring of participants results in fewer hospitalizations, less reliance on prescription medications, better quality of life and better overall health.
PACE Provides Transportation
Getting to and from PACE is easy as all medically-necessary transportation to our PACE centers is provided. Van services can also take you to your appointments at other locations in the community.
The cost, if any depends on whether a participant is eligible for Medicare or Medicaid programs.
If at any time a participant is place in a nursing home, they may be responsible for a portion of the cost or care
