Diabetes and Nutrition Center Services

Diabetes Self-Management Training Program (DSMT)
Our Lady of the Lake first established a diabetes support program in 1981. Our program has expanded over the years to provide comprehensive services for those with diabetes. DSMT is provided to clients with all types of Diabetes (Type 1, Type 2, secondary and gestational) and our team of certified diabetes educators, registered nurses and registered dietitians provide education and ongoing support so you can have a better understanding of how to live with diabetes. With a referral from your physician, you’ll receive an initial assessment, knowledge/skill evaluation, an educational plan with measurable learning and behavioral objectives and insulin pump training (if applicable).

Medical Nutrition Therapy (MNT)
Registered dietitians are available to provide individual medical nutrition therapy to people with nutrition-related problems, such as diabetes, high blood pressure, high cholesterol, kidney disease and weight management (including bariatric clients). This outpatient service provides an opportunity to outline a nutrition program that encourages long-term lifestyle changes to meet health goals.

Diabetes Education and Awareness Group (Support Group)
There are so many issues to consider when living with diabetes. The Diabetes & Nutrition Center’s experts address all these issues in our free monthly Diabetes Education and Awareness Group, which those with diabetes and their families are invited to attend. Topics range from the latest in diabetes equipment to healthy recipes. The diabetes support group meets on the second Tuesday of each month in the Our Lady of the Lake Auditorium from 6 p.m. to 7 p.m.

Physician referrals are required for enrollment in the DSMT and MNT programs. For information on how to obtain a referral, contact our office at (225) 765-8020. Referred individuals will be contacted by our staff to arrange appointments. ​
